Pharmacist in the Practice

 

At Main Street Medical Centre, improving health outcomes for our patients and the wider community is our top priority. The addition of a Pharmacist to our team will support Practice capacity and quality patient care by promoting a team-based approach.  We support the integration of Pharmacy services through the development of strong working relationships between General Practice and community Pharmacy, assisting Practices to support vulnerable patients and reduce preventable hospitalisations. The collaboration between our GPs, Nurses and Practice Pharmacist will help target and identify high-risk patients of medication misadventure.

Benefits of adding a Pharmacist to our Practice team include:

  • identification and reduction of medicine-related problems
  • patient compliance to treatment and appropriate prescribing
  • reduction of the total number of medications patients need to take

We are currently running our Winter Better Health Initiative – all patients who suffer from asthma and COPD are eligible for a free health education session and medication review when they attend a Flu or COVID-19 vaccination. For more information, see here.
